Non-profit organizations, please use the Milford Alive Non-Profit Vendor Application.  Please read the PDF application form below for additional information.  You will still need to SIGN a completed Vendor Application with the Milford Merchants Association (MMA) before setup of the event, see link below for the PDF form that can be printed.

Print the PDF form, complete it, sign it, and either mail it to or drop it off in person at Mystique in Milford.

NOTE: Food Vendors must complete the Municipal Temporary Food Concession Application and submit to Milford Boro before the event. If you are cooking anything, you must also complete a Milford Borough Fire Permit.
